Lock Out Tag Out (LOTO)

BACKGROUND:

Tag Out & Lock Out (LOTO) adalah prosedur keselamatan kerja yang bertujuan untuk melindungi pekerja dari potensi bahaya akibat pelepasan energi yang tidak terduga saat melakukan perawatan atau perbaikan peralatan. LOTO tidak hanya berlaku untuk tim Electrical, tetapi juga penting bagi tim Production, Operation, Maintenance, dan Instrument, karena setiap sistem dalam industri Oil & Gas memiliki potensi bahaya energi yang tinggi.

Namun, dalam banyak kasus, implementasi LOTO sering kali tidak dilakukan secara konsisten atau kurang dipahami oleh semua tim lintas disiplin. Hal ini dapat mengakibatkan insiden yang membahayakan keselamatan pekerja dan lingkungan. Oleh karena itu, pelatihan ini dirancang untuk memberikan pemahaman praktis dan mendalam tentang prosedur LOTO yang dapat diterapkan di berbagai bidang operasi.

OBJECTIVES:

  • Memahami prinsip dasar dan regulasi yang mendasari prosedur LOTO.
  • Mengidentifikasi potensi bahaya energi di lingkungan kerja lintas disiplin.
  • Menguasai penerapan praktis prosedur LOTO untuk semua sistem operasi, termasuk produksi, perawatan, kelistrikan, dan instrumentasi.
  • Menjamin keselamatan kerja melalui pemahaman yang seragam antara tim operasi dan perawatan.
  • Membentuk budaya keselamatan yang kuat melalui implementasi LOTO yang konsisten dan efektif.

 

COURSE OUTLINE:

  1. Dasar hukum dan regulasi yang mendukung LOTO:
    • OSHA 1910.147 dan standar lainnya.
  1. Konsep energi berbahaya dan identifikasinya:
    • Energi mekanik, listrik, hidrolik, pneumatik, termal, dan kimia.
  1. Prinsip dasar Tag Out & Lock Out.
  2. Perbedaan antara LOTO untuk kelistrikan, operasi, dan instrumentasi.
  3. Identifikasi peralatan dan perangkat LOTO yang sesuai.
  4. Langkah-langkah prosedural LOTO:
    • Persiapan, penguncian, penandaan, verifikasi, dan pelepasan.
  5. Simulasi penerapan LOTO pada berbagai jenis sistem:
    • Sistem produksi, sistem mekanik, dan panel listrik.
  6. Penanganan LOTO pada situasi darurat atau khusus.
  7. Peran komunikasi tim lintas disiplin dalam memastikan keberhasilan LOTO.
  8. Audit dan inspeksi implementasi LOTO:
    • Checklist dan laporan kepatuhan.
  1. Studi kasus insiden terkait LOTO:
    • Penyebab, dampak, dan langkah mitigasi.
  1. Latihan praktik kelompok untuk penerapan LOTO di lingkungan kerja yang berbeda.
  2. Pengembangan budaya keselamatan melalui LOTO.

ABOUT THE COURSE LEADER:

Ir. H. Irawadi Prihaswan, MK3, Dipl. Geoth

Mr. Irawadi is professional practitioner with more than 30 years of experience in wide range of working environments; oil & gas, geothermal & mining. His career dedication in Health & Safety Management started in 1999 which is 20 years ago. He accountable for the position of Manager of Emergency Response in Chevron stationed in Balikpapan and then Pekanbaru since 2009. Due to his qualifications and capabilities, he is still now handling advisory position in HSE in one of well-known geothermal company reporting the VP Operation & Maintenance.

He is the manager fully in charge for development & roll-out of ERP/ECS in the company he worked for.

In 1986 he was graduated from Gajah Mada University, received his diploma degree from Auckland, New Zealand. His Master Degree (S2) is in Occupational Health & Safety from University of Indonesia in 2007.

During his careers, he is also internal trainer for the health & safety related matters & shared his knowledge & skills to his community.
